Referrals for Services

If you would like to refer someone for services, please complete the appropriate contact form, for either psychotherapy or evaluation services, and provide your contact information in the comments section for follow-up regarding the referral. To allow for communication once an individual becomes an active client, please complete an authorization for release of information (link below) and either fax to 336-276-1516 OR send via secure e-mail to accounts@carolinapsychologygroup.com.
